Why does the forearm band didn't appear even though I've already activated it?

I'm guessing it's the result of the custom tattoo patch you've using for Andersen.

Sir it's not just for andersen but for all players.....

Right now I'm just using the customized tats by patchers in 256 resolution..

But when I installed the UPB it fix the problem but another problem occur, when I call a timeout it crashes. I think it's because of my 256 tats....

Well, you didn't mention the problem wasn't just with Andersen. If you're using a lot of tattoo patches and those players are having similar issues then I'd certainly agree the problem is likely the tattoo updates.
