Trying to find a Create A Player Template Doc.

Do anybody know where I can find a template for Create A Player? I'm taking about the information, abilities, Vitals, tendencies,Hotspots,abilities,and shots. I'm trying to find an easier way to get my players ready to be created. Thanks in advance for any help.
