If five years is long enough to consider a game old, then NBA Live 06 is one of the first games that comes to mind on all three counts for me. As I just posted in the "Games you're currently playing" thread, I'm still planning on picking it up again at some point and finishing my current season as I've really enjoyed that Dynasty.
Super Mario Bros 3 and Castlevania III on the NES are two old favourites that I'll have a stab at from time to time. I've finished SMB3 on the SNES (as part of the Super Mario All-Stars cart) but never finished the NES version, nor have I ever finished Castlevania III. Sid Meier's Pirates! is one of my all-time favourites, these days I generally play the 2004 remake (which again some might consider old at this point, given that it's over five years old).
Legend of Zelda: Ocarina of Time along with Majora's Mask are two others I've played through quite a bit and will play around with from time to time. Having finished them both a couple of times, these days I tend to look for stuff I've overlooked on past playthroughs and try for a bit of sequence breaking. THQ's wrestling games on the N64 (both the WWF and WCW titles) are others I might play for a bit of nostalgia with a match here or there when the mood strikes. It's been a while since I played the older PS2 WWF/WWE games though. GoldenEye, Duke Nukem 3D and Doom II are some first person shooters I'll sometimes go back and play. Commander Keen too, if I've got some time to kill and I really want to go old school. Grand Theft Auto, mostly III and the games based on that engine. And of course, the early games in the Mortal Kombat series.
That's about all that comes to mind for the moment, there's a lot more games that I'd rate amongst my all-time favourites that I mentioned in the Video Game Hall of Fame thread we had some time back but I don't play them quite as much when I'm in the mood for an old favourite.

Wow, I remember getting the Xmas edition when I was like 5 or 6.
So many of those DOS games were awesome fun. I used to spend hours playing Stunts and Wolfenstein 3D.
And the graphics hold up so well compared to today's games...

I remember playing Twisted Metal Black at one point. I understand why Lamrock loves Twisted Metal, addicting as hell.
Guitar Hero III is the only game I treasure, which I can't play because the red and blue on my guitar don't work.

Jon's post in the other thread reminded me of Donkey Kong Country and its sequels on the SNES, also some of my all-time favourites that I'll play again every so often when I'm in the mood for some old school gaming.
