Sorry, I forgot to upload the ones with instructions but here it is ::

Installation:

1.Enable Custom Art

Go to C:\Program Files\EA SPORTS\NBA Live 2004\main\cscripts,
Open xstartup.cfg with notepad, scroll down to the bottom of
the text, and replace the line "customart 0" with "customart 1"

2.Extract the files in C:/Program Files/NBA Live 2004/
3.Run install.bat
4.If it still doesn't show up in the jersey selection screen, try unlocking it at NBA Store.
