Seems to be an issue with the headshape and in the case of the last one, the skin tone of the new face doesn't match the skintone of the original face. Beyond that, I can't really offer any pointers as I don't make face patches.

is that Dirk's headshape on the first and second picture? as what Andrew pointed out, wrong choice of headshape. and i also think, it's because of misaligned edited CF. also, try to improve the blending to match the skintone. there are so many ways you can match them in photoshop.

as the guys said previously you need to align everything perfectly or else ur gonna have ur cyberfaces end up lookin like that... ill put together a quick tutorial for u on how i do my updates.....
