Unlimited Stamina

I find that the stamina of my players and even computer controlled players do not decrease through the whole game (48 mins). So there is no substitution in the whole game. The 10 starters play all 48 mins.
Why stamina unchanged? Is it a bug? Thanks.

No .. thats no bug.
Go to Options -> Gameplay settings -> Global sliders. There is a slider for fatigue effect. Set it anywhere from 0 to 100. The higher the number, the more fatigued your players will get.

The fatigue bar option is in the In-Game Settings menu.
