Also, the player I am using just stands there during a missed free throw while the opponent goes right around him even if I'm pushing on the control stick. And sometimes my players don't sub in even if it is a free throw or throw in...

Both are issues that have been reported to EA Sports (see the thread in NBA Live 08 General for more info). A few people have tried their hand at editing the sim engine files but there doesn't seem to be any way to resolve the control issue during free throws.
