How Do You Mod Jerseys?

Hi,
Is there a tutorial, or a template, or any general knowledge that anybody could share about how to mod custom jerseys? I have been looking around for a while now and haven't had much luck.
Best i could do was open the dds files but im not sure if there is an easier way.
