Bald, hair but bald patch, afro with sideburns, or afro with beard?

When I think Kareem I think bald. When I think Alcindor I think afro with sideburns.

Very short hair with a nice moustache and those freaky googles. Did he need them actually because of eye injuries or sumthing like that?

Yes.
Once he joined the Lakers, Abdul-Jabbar began wearing his trademark goggles. Years of battling under NBA backboards, and being hit and scratched in the face in the process, had taken their toll on his eyes and he developed corneal erosion syndrome, where the eyes begin to dry out easily and cease to produce moisture. He once missed a game in the 1986-87 season due to his eyes drying out and swelling as a result.

I knew it before hand but it took me about that time to Wiki and paste it.

Thought I'd include the details instead of just answering.

Same. I'm guessing this is for an NBA Live patch, so I'd go with the look he had in the 80s...probably the early 80s when he still had a little hair.

Two images come to mind usually.
Lakers Kareem - short hair, goggles.
Unstoppable Kareem - hookshot over Wilt.

The best scene that comes to my mind with his short hair and googles was when he and Larry Bird were yelling at each other after Kareem got fouled hard in a game against the Celtics.
I think in the same game there was the fight between Kurt Rambis and a player of the Celtics in which Kurt Rambis fell into the media guys behind the basket.

The most devastating weapon in NBA history, a thing of beauty to watch. And he could do it with either hand! According to Kareem, he learned it in fifth grade. It's a shame more big men don't use it.
When I think of Kareem, it's always with the short hair and goggles.
Hova, that was Kevin McHale who clotheslined Kurt Rambis in the thrilling '84 Finals. Lakers-Sixers, Lakers-Celtics, Lakers-Pistons, Celtics-Sixers, Celtics-Pistons - shit it didn't get any better than that during the 80s.
