My setup:
Shoepack + Settings 2.5 + uninjured roster 3.0.5.1 + Sliders (No Version)
I adjusted the sliders to increase user defense. I am not a great shooter, so I also increased user shot success. I haven't quite got that right, but +5-10 compared to CPU is good for me. I adjusted the settings to put speed to 45, might go back down to 40 and see if there is an impact. I play pro mode 6 minute quarters.
Put Settings + uninjured roster + Sliders in your 2k9 saves folder:
C:\Documents and Settings\ %username% \Application Data\2K Sports\NBA 2K9\Saves
Shoepack iff files go in the main 2k9 folder (i assume, never checked to see if it worked).

Settings 2.6 + 3.0.5.1 uninjured Roster + Sliders (Newer)

Not sure what the difference between the settings and sliders might be. I will probably give these a try, but I believe the bulk of the change to the game play is in the roster file (player skills and coach sliders) and then you can tailor the game to your style from there using the settings and sliders.
