Just another NLSC Dynasty

Since i just got an idea to make an NBA Live 2004 last dynasty, it didn`t work out. The game fucked the computer up ( it`s an original version), so i can`t play it anymore. All the NLSC-ers , who joined my dynasty, don`t worry , i will create you for my 2005 dynasty.
Anywayz, the dynasty is with Utah Jazz, and everybody can be in it!
I need your team (in a team there could be maximum 4 created players),birth details, your number, your cyberface( don`t worry, if you don`t have it ,PM me the description), name of course, and birthplace. Also if i forgot something important , you may add it. I am getting the game Tuesday so i guess, it starts at Wednesday or something.
