I play with the keyboard. I don't use any gamepad.
Here's how I configured it.

directions = 8, 4, 6, 2 on the NUMPAD.
jumpshot = R
dunk/layup = F
alley-oop = G
pass = D
crossover = S
Prohop = A
turbo = Left Shift
Backdown = E
Directpass = right arrow (I use my right thumb to press this)
Playcalling = numpad Enter

I've won 3 of my 10 games so far on Allstar, 12min quarters.
I don't have any superstar on my team. I play mostly with:
Starters: Dampier, B Grant, S Battier, E Jones, Rafer Alston.
Bench: Etan Thomas, Eric Williams, Robert Pack, H Grant, Kareem Rush, V Stephania, and H Davis.

It's not really difficult. You just have to work harder.
There's not much Freestyle controls so you rely on defense and simple plays.
