The best way to make a vector is through Adobe Illustrator and it takes a load of time and a huge amount of paitience. If you have dont have the time nor the paitience then heres how you fake a vector to get a simliar result.
FIRST STEP
Open up the picture you want to apply the vector on.
I chose Kevin Garnett pointing.
SECOND STEP
Check Filter > Artistic > Poster Edges
Leave the settings as they are.
THIRD STEP
Click Filter > Artistic > Cutout and add the following settings: (Levels 8, Edge Simplicity 1, Edge Fidelity 3) or adjust the levels to suit you.
The result should be a cool and simple looking effect.
Results will vary as it applies on the pic.

The only way to get the results you want is to actually use original method of creating vector art. I've tried and tried many other methods and they just don't get the job done.

Nope, not really.
A Vector is an image that doesn't use pixels to map out it out. It uses formulas to detirmine where boundaries between colours and gradients belong. Because of this it can be re-sized to any size retaining it's quality. Therefore it's the desired format for any basic print advertising or making banners. On the downside it can't have as much depth as a a typical 'raster' (that's the pixel mapping) image.

I think it looks better as a fake vector when you apply the cutout filter to a effect layer on top of the pic.
Eg. when I duplicated a pic layer and put it to 'Hard Light' and desaturate, then apply the cutout to that layer. Looks better IMO.
I've overdone it here since I put another Cutout'd layer at 60% opacity in between the pic layer and the first Cutout layer:
ps. you might've seen this. The head is messed, but I'm pretty proud about the rest

Yeah it works best when you do cutout to an image, then duplicate the cutout'ed layer and apply some blending option effects and lower opacity until it looks good.
That's how I usually do it, use multiple layers with cutout and different blending modes.
