I've always wondered how common these little accidents happen to a player who played for a team a long time and then gets traded/etc. the first time he plays his old team if their jerseys/players are the same.

That's no miscommunication. You don't set picks for the ballhandler when you're on defense, which is exactly what LeBron does there. That's not a double team, or a mistaken defensive rotation; that's a pick.
