only suggestion I can give is to try and use camera angles that don't give the missing head glitch.

I found that there is usually 1 angle where the heads wont disappear, its just a matter of finding it for that particular play....big pain in the ass though, maybe 4th time will be a charm next year.

Haha! Yeah I can't believe 2k hasn't fixed this yet. I'll spend more time next time, I was pretty anxious to start working on this, I'm sure that hindered this quite a bit.

I used 3 different melos. One was a MP patch that can be found on nba-2k, there's a few of them so just depends on which one you like best. Another was by URH:

And the 3rd one was ***I think*** was this one http://downloads.moddingway.com/file/5765.html by GH33, not 100% sure though.

Rangez, its harder to say, different effects work in different places. I like the sin city and different levels of brightness the best. I use solely Sony Vegas 9.

Look for a tut on Youtube. Its not a built in effect, you have to do some different effects to make it work.
