Try
GPU_LOGICAL_THREAD = 1
or
MAIN_LOGICAL_THREAD = 1

Lowering these two will get your some extra frames at the cost of some jaggies.

VIDEOSETTING_MSAA = 4
VIDEOSETTING_MSAAQUALITY = 2

You should be fine just setting the second one to 0.

This helped me actually. Even though my game was running fine before, but with some stuttering going on sometimes, this helped smooth it out for me. And, there's no noticable deterioration in graphics (don't tell me if there is, I tend to nickpick alot :D )

Man my specs is

GeForce 8500 GT 512MB
AMD Athlon 64 X2 Dual Core 5000+ (2.6GHz)
2GB RAM

and i have problems with framerate..during the game or calling a timeout.. its freeze a few seconds really troublesome..

VIDEOSETTING_MSAA = 8

Try turning that down. Start by going to 4, then to 2. It will cause more aliasing(jagged edges), but it might help.

If that doesn't work, try some of the suggestions from the posts above.

I just don't know what to do anymore.
I've tried everything and nothing works.
My specs are Phenom II x2 3.1GHz, Radeon HD4870 1GB, 2GB Ram and Win7.
On that hardware i should run well so why it doesn't? cfg modifications don't work, 2k9 cfg doesn't work, the drivers are up to date, official patch installed.
What else can be done to raise FPS?
Lowering graphics quality to medium doesn't do the trick and I don't want to play with low details.

You should be running the game great. You can always try to run the game in compatability mode for Vista or XP. Do you have any other games with low performance?

When I turn on V sync my framerate drops from 45-50 range to 30. Anyway to fix this because I know V sync helps with easy lay ups that are air balled badly.

**you can also set the priority of the cpu to high - manually or set it automatically by creating a batch file- start /high nba2k10.exe using notepad and save it as .bat on your 2k10 folder and make a shortcut to desktop then just change the icon to 2k10
**disable visual themes and desktop composition
**try to overclock your GPU and CPU if it's not
**don't turn ON VSYNC via game settings turn it off
Just use direct3Doverrider were it's included when you download Rivatuner 2.24 and then turn ON VSYNC and triple buffering from it. turning ON the triple buffering helps to improve game performance when VSYNC is also turned ON

** if you're using NVIDIA drivers, from the control panel, Program Settings add nba2k10 and set the maximum pre-rendered frames to 2 (the default is 3) Reducing helps if you experience delay on keyboard or controller -I dunno on ATI control panel if it has

** not sure about this but I don't believe at the benchmark of nba2k10 maybe it's not accurate (e.g if the average FPS is 45-50 = maybe the real average is 55=60
try using other benchmark tool like FRAPS etc.

What programs are running in the background?

Check in task manager for programs that have a high Memory or CPU usage

ei guys...do you have suggestions why the game runs slow on my rig with high settings with this specs? my friend has the same specs but the game runs smooth on his system...

processor: intel core2duo 2.20GHZ
ram: 2gb
graphics: ati radeon HD 4670 1GB
and lots of HD space

click properties on nba.exe than go to Compatibility and put ticks on these.
